1. Facilitate smooth transition of patients from hospital to their next level of care.
2. Coordinate with healthcare team to ensure timely and appropriate discharge planning.
3. Conduct thorough assessments of patients' needs and develop individualized care plans.
4. Collaborate with patients, families, and caregivers to provide education and support during transition process.
5. Monitor and track patient progress to identify any potential barriers or issues.
6. Communicate effectively with healthcare team, patients, and families to ensure continuity of care.
7. Work closely with community resources and external agencies to facilitate post-discharge care.
8. Advocate for patients' needs and ensure their preferences are considered in discharge planning.
9. Maintain accurate and up-to-date documentation of patient care and discharge plans.
10. Participate in interdisciplinary meetings and provide input regarding patient needs and discharge plans.
11. Remain current on healthcare policies and regulations related to discharge planning.
12. Demonstrate compassion, empathy, and sensitivity while working with patients and their families.
13. Display excellent organizational and time-management skills to effectively manage a high caseload.
