Social Worker - Long Term Care

LifeBridge Health is seeking an experienced and compassionate Social Worker for our Long Term Care program. The ideal candidate will have a passion for providing support to elderly patients and their families, and a commitment to providing excellent healthcare services.The Social Worker will be responsible for providing support services to elderly patients and their families. This role requires the ability to assess and identify the needs of the patient and family, create plans to meet those needs, and develop an individualized care plan. The Social Worker must be knowledgeable of the regulations, policies, and standards of care related to long term care, and be skilled in building relationships with the patient and family.The successful candidate must have a Master’s degree in Social Work, be a Licensed Social Worker in the state of Maryland, and possess at least two years of experience working with elderly patients and their families in a long term care setting.
