RN Manager - Care Coordination

2. Manage a team of Care Coordinators, providing guidance, support, and mentorship to ensure high-quality patient care.
3. Develop and implement protocols and procedures to improve patient outcomes and streamline processes within the department.
4. Utilize strong clinical expertise to assess patient needs and develop individualized care plans in collaboration with the interdisciplinary team.
5. Monitor and evaluate the performance of Care Coordinators, providing feedback and implementing corrective actions as needed.
6. Collaborate with other department managers and leaders to ensure seamless transitions of care and promote interdisciplinary communication.
7. Act as a liaison between the Care Coordination department and other departments within the healthcare organization, fostering a collaborative and cohesive work environment.
8. Ensure compliance with all regulatory and accreditation standards, as well as organizational policies and procedures.
9. Identify opportunities for process improvement and implement strategies to enhance patient care delivery.
