
RN Manager - Care Coordination

- Oversee the day-to-day operations of the Care Coordination department, ensuring efficient and effective delivery of patient care services.
- Manage a team of Care Coordinators, providing guidance, support, and mentorship to ensure high-quality patient care.
- Develop and implement protocols and procedures to improve patient outcomes and streamline processes within the department.
- Utilize strong clinical expertise to assess patient needs and develop individualized care plans in collaboration with the interdisciplinary team.
- Monitor and evaluate the performance of Care Coordinators, providing feedback and implementing corrective actions as needed.
- Collaborate with other department managers and leaders to ensure seamless transitions of care and promote interdisciplinary communication.
- Act as a liaison between the Care Coordination department and other departments within the healthcare organization, fostering a collaborative and cohesive work environment.
- Ensure compliance with all regulatory and accreditation standards, as well as organizational policies and procedures.
- Identify opportunities for process improvement and implement strategies to enhance patient care delivery.
- Serve as a role model for ethical and professional behavior, promoting a positive work culture and upholding the organization's mission and values.
Bachelor's Degree In Nursing Or Related Field, With A Current Rn License In The State Of Employment.
Minimum Of 5 Years Of Experience In Nursing, With At Least 2 Years In A Management Or Leadership Role.
Strong Knowledge Of Healthcare Regulations, Policies, And Procedures, Particularly In Regards To Care Coordination And Case Management.
Demonstrated Ability To Lead And Manage A Team, Including Providing Coaching And Mentorship To Staff.
Excellent Communication And Interpersonal Skills, With The Ability To Collaborate And Build Relationships With Various Stakeholders, Including Patients, Families, Physicians, And Other Healthcare Professionals.
