Accurately Collect and Verify Medication History: The Medication History Pharmacy Technician will be responsible for collecting and verifying patients' medication histories through various methods such as interviewing patients, reviewing medical records, and communicating with other healthcare providers.
  2. Document Medication History: It is the responsibility of the Medication History Pharmacy Technician to accurately document all medication information gathered from patients and other sources in the electronic health record system.
  3. Ensure Patient Safety: The primary responsibility of the Medication History Pharmacy Technician is to ensure the safety of patients by identifying and resolving any discrepancies or potential issues with medication histories.
  4. Communicate with Patients and Healthcare Providers: The Medication History Pharmacy Technician will be responsible for communicating with patients and healthcare providers to gather and verify medication information, as well as provide updates and clarification as needed.
  5. Attention to Detail: The Medication History Pharmacy Technician must have a keen eye for detail and be able to accurately and thoroughly document medication information to ensure the safety and effectiveness of treatment.
  6. Maintain Confidentiality: It is the responsibility of the Medication History Pharmacy Technician to maintain patient confidentiality and adhere to all HIPAA regulations when handling medication information.
  7. Collaborate with Healthcare Team: The Medication History Pharmacy Technician will work closely with pharmacists, nurses, and other healthcare professionals to ensure accurate medication histories and resolve any issues that may arise.
  8. Adhere to Policies and Procedures: The Medication History Pharmacy Technician is responsible for following all policies and procedures set by the organization and maintaining compliance with all state and federal regulations.
