INPATIENT SOCIAL WORKER LCSW/LMSW

1. Provide direct social work services to inpatients and their families, including psychosocial assessments, counseling, and referrals to community resources.
2. Collaborate with interdisciplinary healthcare team members to develop and implement holistic treatment plans for patients.
3. Advocate for patients' rights and needs, ensuring that their physical, emotional, and social needs are met during their hospital stay.
4. Utilize knowledge and understanding of social work principles and ethics to promote culturally competent and client-centered care.
5. Conduct thorough assessments to identify patient and family strengths, needs, and barriers to care, and develop appropriate interventions.
6. Maintain accurate and timely documentation of all social work services provided, including treatment plans, progress notes, and discharge plans.
7. Coordinate and facilitate support groups and educational programs for patients and their families.
8. Collaborate with community organizations and agencies to develop and maintain a network of resources for patients.
9. Provide crisis intervention and support to patients and families during times of emotional distress or difficult decisions.
10. Participate in continuous quality improvement initiatives to enhance the delivery of social work services within the hospital.
11. Serve as a liaison between patients, families, and hospital staff to ensure effective communication and coordination of care.
12. Maintain confidentiality of all patient information and adhere to all HIPAA regulations.
13. Stay current with industry trends, developments, and best practices in social work and healthcare.
