- Serve as a primary point of contact for hospice care providers, building and maintaining strong relationships to ensure seamless coordination of care for patients.
- Collaborate with interdisciplinary teams to identify patients who may benefit from hospice services and facilitate appropriate referrals.
- Conduct initial assessments of patients' hospice needs and work with hospice providers to develop individualized care plans.
- Provide education to patients and their families about hospice care and assist in addressing any questions or concerns.
- Advocate for patients' rights and ensure that all necessary paperwork and authorizations are completed accurately and in a timely manner.
- Coordinate and schedule hospice consultations and visits, ensuring that all parties are informed and prepared for each visit.
- Monitor and track patient progress and communicate any changes or concerns to the hospice team and the patient's primary care team.
- Collaborate with insurance companies and other third-party payers to obtain necessary authorizations for hospice services.
- Maintain accurate and up-to-date records of all hospice-related activities and communications.
- Participate in regular team meetings and provide updates on hospice referrals and patient outcomes.
- Continuously evaluate and improve hospice liaison processes and procedures to ensure efficient and effective coordination of care.
- Uphold the values and mission of LifeBridge Health by providing compassionate and personalized care to patients and their families.
- Adhere to all relevant legal and ethical standards and maintain confidentiality of patient information at all times.
- Attend professional development and training opportunities to expand knowledge and stay current on hospice care practices and regulations.
- Perform any other duties as assigned to support the overall goals and objectives of the hospice program and LifeBridge Health.
Excellent Communication And Interpersonal Skills.
Bachelor's Degree In Healthcare, Marketing, Or Related Field.
Minimum Of 2 Years Experience In Hospice Or Healthcare Sales.
Strong Understanding Of Hospice Regulations And Reimbursement Processes.
Ability To Build And Maintain Relationships With Referral Sources And Community Partners.
