HOSPICE AIDE

1. Provide direct patient care and emotional support to hospice patients and their families during their end-of-life journey.
2. Assist patients with activities of daily living, including bathing, grooming, and dressing.
3. Monitor and record patient vital signs, intake and output, and changes in condition.
4. Administer medications and treatments as directed by the hospice nurse.
5. Communicate effectively with patients, families, and interdisciplinary team members to ensure the best possible care for each patient.
6. Maintain a clean and safe environment for patients, following infection control and safety protocols.
7. Document patient care accurately and timely in accordance with hospice policies and procedures.
8. Provide companionship and emotional support to patients and their families.
9. Attend and actively participate in team meetings, in-services, and educational programs.
10. Collaborate with other healthcare professionals to ensure a holistic approach to patient care.
11. Provide end-of-life education and support to patients and their families.
12. Demonstrate sensitivity and respect towards patients, families, and co-workers.
13. Adhere to all state and federal regulations, as well as hospice policies and procedures.
14. Continuously strive to improve patient care and promote a positive work environment.
15. Maintain confidentiality of patient information and adhere to HIPAA regulations.
