  1. Develop and implement strategies to improve the quality and effectiveness of the Diabetes Self Management Program.
  2. Coordinate and oversee all aspects of the program, including scheduling classes, coordinating with healthcare providers, and managing resources.
  3. Conduct assessments to determine the individual needs of patients and develop personalized care plans.
  4. Educate patients on diabetes management techniques, including monitoring blood sugar levels, healthy eating habits, and medication management.
  5. Provide ongoing support and resources to patients to help them successfully manage their diabetes and make healthy lifestyle changes.
  6. Collaborate with healthcare professionals to ensure a comprehensive approach to patient care.
  7. Keep accurate records of patient progress and program outcomes.
  8. Stay updated on the latest research and developments in diabetes management to continually improve the program.
  9. Advocate for the program and promote it within the community to increase awareness and participation.
  10. Maintain a high level of professionalism and compassion while working with patients from diverse backgrounds.
  11. Ensure compliance with all relevant healthcare regulations and guidelines.
  12. Collaborate with other healthcare professionals and support staff to ensure seamless delivery of care.
  13. Continuously evaluate the program and make necessary adjustments to meet the needs of patients and improve outcomes.

Job Qualifications
  • Bachelor's Degree In Nursing, Social Work, Or A Related Field.

  • Minimum Of 2 Years Of Experience In Diabetes Education And Self-Management.

  • Certification As A Diabetes Educator (Cde) Preferred.

  • Strong Knowledge Of Diabetes Management Techniques, Including Medication Management, Nutrition, And Exercise.

  • Excellent Communication And Interpersonal Skills To Effectively Educate And Support Patients In Self-Management Techniques.
