- Develop and implement strategies to improve the quality and effectiveness of the Diabetes Self Management Program.
- Coordinate and oversee all aspects of the program, including scheduling classes, coordinating with healthcare providers, and managing resources.
- Conduct assessments to determine the individual needs of patients and develop personalized care plans.
- Educate patients on diabetes management techniques, including monitoring blood sugar levels, healthy eating habits, and medication management.
- Provide ongoing support and resources to patients to help them successfully manage their diabetes and make healthy lifestyle changes.
- Collaborate with healthcare professionals to ensure a comprehensive approach to patient care.
- Keep accurate records of patient progress and program outcomes.
- Stay updated on the latest research and developments in diabetes management to continually improve the program.
- Advocate for the program and promote it within the community to increase awareness and participation.
- Maintain a high level of professionalism and compassion while working with patients from diverse backgrounds.
- Ensure compliance with all relevant healthcare regulations and guidelines.
- Collaborate with other healthcare professionals and support staff to ensure seamless delivery of care.
- Continuously evaluate the program and make necessary adjustments to meet the needs of patients and improve outcomes.
Bachelor's Degree In Nursing, Social Work, Or A Related Field.
Minimum Of 2 Years Of Experience In Diabetes Education And Self-Management.
Certification As A Diabetes Educator (Cde) Preferred.
Strong Knowledge Of Diabetes Management Techniques, Including Medication Management, Nutrition, And Exercise.
Excellent Communication And Interpersonal Skills To Effectively Educate And Support Patients In Self-Management Techniques.
