
Community Support Specialist

- Engage and build relationships with individuals and families in the local community to understand their needs and provide support.
- Develop and implement community outreach programs and activities to promote health and well-being.
- Collaborate with other community organizations and agencies to coordinate services and resources for individuals and families.
- Conduct needs assessments and create individualized support plans for clients.
- Provide crisis intervention and emotional support to individuals in difficult situations.
- Connect clients with appropriate resources such as housing, employment, healthcare, and education.
- Advocate for clients and help them navigate complex systems and processes.
- Maintain accurate and confidential records of client interactions and progress.
- Participate in team meetings and contribute ideas for improving services and support for the community.
- Stay up-to-date on community resources and services to better assist clients.
- Attend trainings and workshops to enhance knowledge and skills in providing support to individuals and families.
- Uphold ethical and professional standards in all interactions with clients and the community.
- Communicate effectively with clients, colleagues, and other stakeholders.
- Represent LifeBridge Health in a positive and professional manner at all times.
- Continuously evaluate and improve services to meet the changing needs of the community.
Bachelor's Degree In A Related Field Such As Social Work, Psychology, Or Counseling.
At Least 2 Years Of Experience Working With Individuals With Mental Health Or Behavioral Health Needs.
Knowledge Of Community Resources And Ability To Connect Clients To Appropriate Services.
Strong Communication And Interpersonal Skills To Effectively Engage With Clients And Collaborate With A Multidisciplinary Team.
Ability To Work Independently And Handle Crisis Situations With Calmness And Professionalism.
