Job Description

We are looking for an enthusiastic Community Support Specialist to join the LifeBridge Health team! This position requires a self-starter that can work independently, as well as in a team environment. Our ideal candidate should have a passion for helping others, be comfortable working with diverse populations, and have a strong commitment to customer service. The successful Community Support Specialist will be able to provide support to our patients and their families, coordinate and facilitate community programs, and develop and maintain partnerships with local organizations. To be considered for this role, you must have a Bachelor’s degree in social work, psychology, or a related field, and at least two years of experience in direct service with individuals and/or families in a health-related setting. The ability to effectively communicate with a diverse group of people is essential, as well as the capacity to work independently and collaboratively. Responsibilities:

  1. Provide support to patients and their families.
  2. Coordinate and facilitate community programs.
  3. Develop and maintain partnerships with local organizations.
  4. Communicate with a diverse group of people.
  5. Work independently and collaboratively.
  6. Maintain patient records and ensure compliance with relevant laws, regulations, and policies.
  7. Assist in the planning and implementation of community outreach activities.
  8. Coordinate and facilitate workshops and special events.
  9. Track patient progress and outcomes.
  10. Provide crisis intervention services, if necessary.
