1. Coordinate care for patients by assessing their needs and connecting them with necessary resources in the community.
2. Serve as a liaison between patients, healthcare providers, and community organizations to ensure effective communication and collaboration.
3. Utilize knowledge of community resources and services to assist patients in accessing necessary support.
4. Develop and maintain relationships with community organizations to expand resources available to patients.
5. Conduct thorough assessments of patients' needs and create individualized care plans.
6. Monitor and track patients' progress and make necessary adjustments to care plans.
7. Educate patients and their families on available resources and how to access them.
8. Participate in multidisciplinary team meetings to discuss patient care and progress.
9. Document all patient interactions and maintain accurate records.
10. Collaborate with insurance companies and other third-party payers to ensure appropriate coverage for patients.
11. Stay up-to-date on industry trends and best practices in community care coordination.
12. Adhere to all ethical and legal standards in patient care.
13. Maintain confidentiality of patient information.
14. Communicate effectively and compassionately with patients from diverse backgrounds and cultures.
15. Provide excellent customer service to patients and their families.
