Clinical Pharmacist - Critical Care

This position requires a professional with excellent patient care skills, exceptional organizational abilities, and a strong commitment to providing excellent service.The successful candidate for this position will have at least two years of experience as a Clinical Pharmacist and must be a licensed Pharmacist in the state of Maryland. The ideal candidate will have a Doctor of Pharmacy (PharmD) degree from an accredited college of pharmacy and have completed a minimum of two years of residency training in a hospital setting. Responsibilities:

1. Provide safe and effective patient-centered care in the Critical Care Department.
2. Ensure compliance with all applicable laws, regulations, and standards related to the practice of pharmacy.
3. Develop and implement patient-specific medication regimens to meet patient needs.
4. Provide education to patients and health care professionals on the safe and effective use of medications.
5. Monitor patient medication regimens for efficacy, safety, and cost-effectiveness.
6. Communicate effectively with other health care professionals and collaborate on patient care decisions.
7. Participate in the development and implementation of policies and procedures related to clinical pharmacy services.
8. Maintain accurate and up-to-date patient records.
9. Participate in quality improvement initiatives.
10. Other duties as assigned.
