Chaplain- Hospice

Welcome to LifeBridge Health! We are currently seeking a compassionate and dedicated individual to join our team as a Chaplain for our Hospice program. As a faith-based organization, we understand the importance of providing spiritual and emotional support to patients and their families during a difficult time. 1. Provide spiritual and emotional support to patients and their families in the hospice program.
2. Conduct regular visits and offer spiritual guidance to patients and their families according to their individual needs.
3. Communicate and collaborate with the interdisciplinary team to ensure holistic care for patients.
4. Participate in interdisciplinary team meetings to discuss patient care and provide spiritual insight.
5. Perform religious ceremonies, rituals, and services for patients and their families as requested.
6. Maintain records and document spiritual care provided to patients.
7. Offer grief counseling and support to patients' families during and after the hospice program.
8. Collaborate with community faith leaders to provide additional spiritual support for patients and their families.
9. Educate and train staff members on spiritual care and end-of-life support.
10. Uphold the values and mission of LifeBridge Health as a faith-based organization.
11. Adhere to all ethical and legal standards related to spiritual care and end-of-life support.
12. Continuously seek opportunities for personal and professional growth in the field of spiritual and end-of-life care.
13. Maintain confidentiality and respect for patients' beliefs and diversity.
14. Participate in quality improvement initiatives and contribute to the development of spiritual care programs.
15. Provide on-call coverage for spiritual support needs outside of regular business hours.
