Job Description

Join our team at LifeBridge Health as a Chaplain for our Bridging Life Hospice program! Our mission is to provide spiritual support and guidance to patients and their families during their end-of-life journey. We are seeking a compassionate and dedicated individual who is passionate about bringing comfort and peace to those facing terminal illness. As a PRN Chaplain, you will have the opportunity to make a meaningful impact on the lives of our patients and their loved ones. If you are a certified Chaplain with experience in hospice care, we encourage you to apply and become a part of our caring and supportive team.

  1. Provide spiritual support and guidance to patients and their families in the Bridging Life Hospice program.
  2. Offer comfort and peace to patients facing terminal illness.
  3. Conduct spiritual assessments and develop individualized care plans for patients.
  4. Collaborate with interdisciplinary teams to address the spiritual needs of patients.
  5. Conduct religious services and provide pastoral care as needed.
  6. Provide emotional and spiritual support to families and loved ones.
  7. Advocate for patients' spiritual needs and preferences.
  8. Offer grief counseling and support to families after the passing of a loved one.
  9. Document all spiritual care interventions and maintain accurate records.
  10. Maintain confidentiality and respect patients' religious beliefs.
  11. Participate in ongoing education and training to stay up-to-date with hospice care practices.
  12. Adhere to ethical guidelines and professional standards of conduct.
  13. Foster a positive and supportive environment for patients, families, and staff.
  14. Serve as a liaison between the hospice program and local faith communities.
  15. Support and promote the mission and values of LifeBridge Health.

Job Qualifications
  • Master's Degree In Divinity, Theology, Or A Related Field From An Accredited Institution.

  • Ordination Or Endorsement By A Recognized Religious Organization.

  • Minimum Of 2 Years Of Experience In Providing Spiritual Care And Counseling To Patients And Families In A Healthcare Setting, Preferably In Hospice Care.

  • Knowledge And Understanding Of Various Faith Traditions And Ability To Provide Culturally Sensitive Care.

  • Ability To Work Independently And As Part Of A Multidisciplinary Team, With Excellent Communication And Interpersonal Skills.
