Our mission is to provide spiritual support and guidance to patients and their families during their end-of-life journey. We are seeking a compassionate and dedicated individual who is passionate about bringing comfort and peace to those facing terminal illness. As a PRN Chaplain, you will have the opportunity to make a meaningful impact on the lives of our patients and their loved ones. - Provide spiritual support and guidance to patients and their families in the Bridging Life Hospice program.
- Offer comfort and peace to patients facing terminal illness.
- Conduct spiritual assessments and develop individualized care plans for patients.
- Collaborate with interdisciplinary teams to address the spiritual needs of patients.
- Conduct religious services and provide pastoral care as needed.
- Provide emotional and spiritual support to families and loved ones.
- Advocate for patients' spiritual needs and preferences.
- Offer grief counseling and support to families after the passing of a loved one.
- Document all spiritual care interventions and maintain accurate records.
- Maintain confidentiality and respect patients' religious beliefs.
- Participate in ongoing education and training to stay up-to-date with hospice care practices.
- Adhere to ethical guidelines and professional standards of conduct.
- Foster a positive and supportive environment for patients, families, and staff.
- Serve as a liaison between the hospice program and local faith communities.
- Support and promote the mission and values of LifeBridge Health.
Master's Degree In Divinity, Theology, Or A Related Field From An Accredited Institution.
Ordination Or Endorsement By A Recognized Religious Organization.
Minimum Of 2 Years Of Experience In Providing Spiritual Care And Counseling To Patients And Families In A Healthcare Setting, Preferably In Hospice Care.
Knowledge And Understanding Of Various Faith Traditions And Ability To Provide Culturally Sensitive Care.
Ability To Work Independently And As Part Of A Multidisciplinary Team, With Excellent Communication And Interpersonal Skills.
