Case Manager Social Worker - LMSW - Grace Medical Center - ED

1. Conduct comprehensive assessments of patients' psychosocial needs and develop individualized care plans to address those needs.
2. Collaborate with interdisciplinary teams to coordinate and implement appropriate interventions for patients in crisis.
3. Provide emotional support and counseling to patients and their families during difficult and stressful situations.
4. Assist patients in accessing community resources and support services, such as mental health resources, housing assistance, and financial aid.
5. Advocate for patients' rights and ensure their needs are being met while they are receiving care at the hospital.
6. Document all interactions and interventions in patients' medical records according to organizational and legal standards.
7. Educate patients and their families on their rights and responsibilities, as well as available resources and support systems.
8. Participate in discharge planning and make appropriate referrals for continued care and support post-hospitalization.
9. Stay up-to-date on current social work practices, policies, and procedures to ensure the highest quality of care for patients.
10. Maintain confidentiality and adhere to ethical standards in all interactions with patients and their families.
11. Collaborate with community organizations and agencies to strengthen partnerships and improve access to resources for patients.
12. Participate in training and professional development opportunities to enhance skills and knowledge in the field of social work.
13. Communicate effectively with team members, patients, and their families to ensure a coordinated and integrated approach to patient care.
14. Adhere to all hospital policies, procedures, and regulations, including safety and infection control protocols.
15. Serve as a positive role model and represent LifeBridge Health and Grace Medical Center in a professional and compassionate manner at all times.
