1. Assist the care management team in their daily tasks and responsibilities.
2. Provide administrative support, including scheduling appointments, maintaining records, and preparing correspondence.
3. Communicate with patients and their families to gather necessary information and assist with any inquiries or concerns.
4. Collaborate with other departments and healthcare professionals to coordinate and facilitate patient care.
5. Maintain confidentiality and adhere to HIPAA regulations while handling sensitive patient information.
6. Participate in team meetings and provide updates on patient cases and progress.
7. Help to identify and address any potential barriers to patient care.
8. Contribute to the development and implementation of patient care plans.
9. Monitor and track patient outcomes and report any significant changes to the care management team.
10. Ensure that all documentation and paperwork is accurate and up to date.
11. Assist with discharge planning and make necessary arrangements for patients transitioning to other levels of care.
12. Continuously strive to improve the quality of care and patient satisfaction.
13. Serve as a liaison between patients, families, and healthcare providers to facilitate effective communication and resolution of any issues.
14. Adhere to all hospital policies and procedures, as well as industry standards and regulations.
15. Maintain a positive and professional attitude while representing LifeBridge Health and its mission to provide exceptional care.
