
Ambulatory Pharmacist

- Dispensing Medications: The Ambulatory Pharmacist will be responsible for accurately and safely dispensing medications to patients in accordance with state and federal laws and regulations.
- Patient Consultations: The Pharmacist will conduct patient consultations to ensure proper medication use, provide education on potential side effects and drug interactions, and address any questions or concerns.
- Collaborate with Healthcare Team: The Ambulatory Pharmacist will work closely with physicians, nurses, and other healthcare professionals to ensure coordinated and comprehensive care for patients.
- Medication Management: The Pharmacist will be responsible for reviewing patient medication profiles, identifying potential drug therapy problems, and making recommendations for therapeutic alternatives or adjustments.
- Compliance and Documentation: It is the responsibility of the Ambulatory Pharmacist to maintain accurate and thorough records of all medications dispensed, patient consultations, and other related activities in compliance with organizational and regulatory requirements.
- Patient Advocacy: The Pharmacist will serve as an advocate for patients, ensuring their needs and concerns are addressed and their rights are respected.
- Quality Assurance: The Ambulatory Pharmacist will participate in quality assurance initiatives to ensure the highest level of patient care and medication safety.
- Continuous Learning: The Pharmacist will stay up-to-date on new drugs, treatments, and regulations through ongoing education and training.
- Teamwork and Communication: The Ambulatory Pharmacist will collaborate and communicate effectively with other healthcare team members to provide seamless and comprehensive care for patients.
- Professionalism: The Pharmacist will maintain a professional demeanor and adhere to ethical standards in all interactions with patients, colleagues, and other healthcare professionals.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited Pharmacy School
Current And Valid State Pharmacy License
Minimum Of 2 Years Of Experience As A Licensed Pharmacist In An Ambulatory Care Setting
Strong Knowledge Of Medications And Their Appropriate Use In Ambulatory Patients
