1. Dispensing Medications: The Ambulatory Pharmacist will be responsible for accurately and safely dispensing medications to patients in accordance with state and federal laws and regulations.
2. Patient Consultations: The Pharmacist will conduct patient consultations to ensure proper medication use, provide education on potential side effects and drug interactions, and address any questions or concerns.
3. Collaborate with Healthcare Team: The Ambulatory Pharmacist will work closely with physicians, nurses, and other healthcare professionals to ensure coordinated and comprehensive care for patients.
4. Medication Management: The Pharmacist will be responsible for reviewing patient medication profiles, identifying potential drug therapy problems, and making recommendations for therapeutic alternatives or adjustments.
5. Compliance and Documentation: It is the responsibility of the Ambulatory Pharmacist to maintain accurate and thorough records of all medications dispensed, patient consultations, and other related activities in compliance with organizational and regulatory requirements.
