We are currently seeking a Temporary Resource Specialist to join our Care Management team. As a Resource Specialist, you will play a crucial role in connecting patients with the necessary resources to support their healthcare journey.   1. Conduct initial screenings of patients to determine their specific needs and connect them with appropriate resources.
  2. Collaborate with the Care Management team to develop individualized care plans for patients.
  3. Utilize a variety of resources, including community organizations and government programs, to identify and obtain necessary resources for patients.
  4. Maintain accurate and up-to-date documentation of all patient interactions and resource referrals.
  5. Communicate effectively with patients, their families, and other healthcare professionals to ensure a smooth and timely delivery of resources.
  6. Stay current on available resources and programs in the community to provide the most comprehensive support to patients.
  7. Advocate for patients and their needs, ensuring they are receiving the best possible care.
  8. Work closely with insurance companies and other third-party payers to obtain coverage for necessary resources.
  9. Attend team meetings to provide updates on patient progress and collaborate on care plans.
  10. Maintain confidentiality and adhere to all HIPAA regulations in handling patient information.
  11. Identify and report any potential issues or concerns with patient care or resource availability.
  12. Provide support and guidance to patients and their families throughout their healthcare journey.
  13. Act as a liaison between patients, healthcare providers, and community resources.
  14. Demonstrate a commitment to providing compassionate care and exceptional customer service to all patients.
  15. Attend training and professional development opportunities to enhance knowledge and skills in care management and available resources.

Job Qualifications
  • Bachelor's Degree In Nursing, Social Work, Or A Related Field.

  • Minimum Of 2 Years Experience In A Healthcare Setting, Preferably In Care Management.

  • Knowledge Of Medicare And Medicaid Regulations And Reimbursement Guidelines.

  • Strong Communication And Interpersonal Skills To Effectively Collaborate With Interdisciplinary Teams And Patients.

  • Ability To Prioritize And Manage Multiple Tasks In A Fast-Paced And Changing Environment.
