  1. Conduct medication reconciliation across different healthcare settings to ensure the accuracy and safety of patient medication orders.
  2. Collaborate with healthcare professionals, including physicians, nurses, and pharmacists, to gather and verify medication information.
  3. Review patient medical records to identify discrepancies and potential medication errors.
  4. Communicate with patients and their families to obtain accurate medication histories.
  5. Enter medication orders into electronic health records and assist with prescription refills.
  6. Handle and store medications according to established protocols and safety standards.
  7. Monitor medication inventory and assist with ordering and restocking as needed.
  8. Maintain accurate and complete documentation of medication reconciliation processes.
  9. Participate in quality improvement initiatives to enhance medication safety and efficiency.
  10. Provide education and training to healthcare staff on medication reconciliation best practices.
  11. Adhere to all state and federal laws and regulations related to medication handling and patient privacy.
  12. Conduct ongoing research and stay informed on changes in pharmaceuticals and healthcare regulations.

Job Qualifications
  • Certification Or Licensure As A Pharmacy Technician: Candidates Should Possess A Valid Certification Or License As A Pharmacy Technician From An Accredited Program, Demonstrating A Strong Understanding Of Medication Management And Reconciliation.

  • Experience In Medication Reconciliation: A Minimum Of 1-2 Years Of Experience In A Pharmacy Or Healthcare Setting, With A Focus On Medication Reconciliation, Is Preferred For This Role. This Experience Should Showcase An Ability To Accurately And Efficiently Reconcile Medication Lists And Identify Discrepancies.

  • Strong Knowledge Of Pharmaceuticals And Medical Terminology: A Thorough Understanding Of Different Types Of Medications, Their Uses, And Potential Interactions Is Necessary For This Role. Candidates Should Also Have A Strong Grasp Of Medical Terminology To Effectively Communicate With Healthcare Professionals.

  • Attention To Detail And Accuracy: The Role Of A Pharmacy Technician In Medication Reconciliation Requires Meticulous Attention To Detail And A High Level Of Accuracy. Candidates Should Have A Proven Track Record Of Consistently Producing Error-Free Work.
