- Manage and lead the Care Management team to ensure high-quality and compassionate care for patients at Legacy Health.
- Develop and implement strategies to improve patient outcomes and satisfaction.
- Monitor and evaluate team performance and provide regular feedback and support for professional growth.
- Collaborate with other departments and healthcare providers to ensure seamless coordination of care for patients.
- Develop and maintain policies and procedures for the Care Management department to ensure compliance with regulatory standards.
- Manage department budget and resources effectively to achieve organizational goals.
- Conduct regular meetings and trainings to keep the team up-to-date on best practices and industry changes.
- Serve as a role model for effective communication and teamwork, promoting a positive and professional work environment.
- Oversee the development and implementation of care plans for patients, ensuring they are individualized and evidence-based.
- Stay informed about current healthcare trends and changes in regulations to continuously improve the Care Management department.
- Foster a culture of continuous improvement by identifying and implementing process improvement initiatives.
- Maintain confidentiality of patient information and adhere to ethical and legal standards.
- Communicate effectively with patients, families, and other healthcare providers to ensure a collaborative approach to care.
- Act as a liaison between the Care Management team and other departments to facilitate collaboration and problem-solving.
- Act as a resource for staff, providing guidance, support, and mentorship to promote professional development.
- Participate in hiring, onboarding, and performance evaluations for the Care Management team.
- Ensure compliance with all safety and infection control protocols.
- Represent the Care Management department in meetings, committees, and other forums as needed.
- Uphold the mission, vision, and values of Legacy Health in all aspects of the job.
- Continuously strive for excellence in patient care and outcomes.
Bachelor's Degree In Nursing, Healthcare Administration, Or A Related Field.
Minimum Of 5 Years Of Experience In Care Management Or Healthcare Management, Preferably In A Hospital Or Healthcare System Setting.
Strong Leadership Skills With A Proven Track Record Of Effectively Managing And Developing A Team Of Care Management Staff.
Knowledge And Understanding Of Healthcare Regulations And Compliance Standards, Including Medicare And Medicaid Guidelines.
Excellent Communication And Interpersonal Skills, With The Ability To Collaborate With Interdisciplinary Teams And Build Relationships With Patients And Families.
