
Foster Care Peer Mentor

- Provide guidance and support to foster care youth and families through regular meetings, phone calls, and other forms of communication.
- Serve as a mentor and role model for foster care youth, helping them navigate challenges and transitions within the foster care system.
- Collaborate with other members of the Legacy Health team to develop and implement individualized care plans for each foster care youth.
- Connect foster care youth and families with necessary resources and services, such as counseling, education, and housing.
- Attend trainings and stay up-to-date on best practices and policies within the foster care system.
- Maintain accurate and detailed records of interactions and progress with foster care youth and families.
- Advocate for the needs and rights of foster care youth and families within the healthcare system and the community.
- Continuously assess and monitor the well-being of foster care youth and intervene as needed.
- Provide emotional support and a safe space for foster care youth to share their thoughts and feelings.
- Participate in team meetings and case conferences to discuss progress and make recommendations for improvements in care.
- Maintain confidentiality and adhere to all ethical and professional standards in working with foster care youth and families.
- Develop and maintain positive relationships with foster care agencies and other community partners.
- Act as a liaison between foster care youth, families, and Legacy Health to ensure effective communication and collaboration.
- Serve as a positive representative of Legacy Health and its values in all interactions with foster care youth, families, and the community.
Knowledge And Experience In The Foster Care System: A Successful Candidate For A Foster Care Peer Mentor At Legacy Health Should Possess A Thorough Understanding Of The Foster Care System, Including Its Processes, Challenges, And Resources. Previous Experience Working With Youth In Foster Care Or Personal Experience As A Former Foster Youth Would Be Beneficial.
Strong Communication And Interpersonal Skills: As A Mentor, It Is Essential To Have Excellent Communication Skills To Effectively Connect With And Support Foster Youth. This Includes Active Listening, Empathy, And The Ability To Communicate In A Non-Judgmental And Supportive Manner.
Empathy And Understanding: In Addition To Having Knowledge About The Foster Care System, A Foster Care Peer Mentor At Legacy Health Should Be Empathetic And Able To Understand The Unique Challenges And Experiences Of Foster Youth. This Will Allow Them To Build A Strong Rapport With Mentees And Provide Effective Support.
Flexibility And Adaptability: The Foster Care System Can Be Unpredictable And Fast-Paced, So A Successful Peer Mentor Should Be Flexible And Adapt Quickly To Changing Circumstances. This May Include Working With A Diverse Range Of Youth And Being Able To Adjust To Different Communication Styles And Needs.
