Foster Care Peer Mentor

1. Provide guidance and support to foster care youth and families through regular meetings, phone calls, and other forms of communication.
2. Serve as a mentor and role model for foster care youth, helping them navigate challenges and transitions within the foster care system.
3. Collaborate with other members of the Legacy Health team to develop and implement individualized care plans for each foster care youth.
4. Connect foster care youth and families with necessary resources and services, such as counseling, education, and housing.
5. Attend trainings and stay up-to-date on best practices and policies within the foster care system.
6. Maintain accurate and detailed records of interactions and progress with foster care youth and families.
7. Advocate for the needs and rights of foster care youth and families within the healthcare system and the community.
8. Continuously assess and monitor the well-being of foster care youth and intervene as needed.
9. Provide emotional support and a safe space for foster care youth to share their thoughts and feelings.
10. Participate in team meetings and case conferences to discuss progress and make recommendations for improvements in care.
11. Maintain confidentiality and adhere to all ethical and professional standards in working with foster care youth and families.
12. Develop and maintain positive relationships with foster care agencies and other community partners.
13. Act as a liaison between foster care youth, families, and Legacy Health to ensure effective communication and collaboration.
14. Serve as a positive representative of Legacy Health and its values in all interactions with foster care youth, families, and the community.
