Responsibilities:

1. Provide direct social work services to patients and their families, including psychosocial assessments, counseling, and crisis intervention.
2. Collaborate with a multidisciplinary team, including physicians, nurses, and other healthcare professionals, to develop and implement individualized care plans.
3. Conduct ongoing assessments to identify patients' needs and ensure the provision of appropriate services and resources.
4. Advocate for patients' rights and needs, including assisting with accessing community resources and navigating the healthcare system.
