  1. Provide emotional support and counseling to patients and their families during their healthcare journey.
  2. Conduct assessments and develop treatment plans for patients' psychosocial needs.
  3. Collaborate with interdisciplinary healthcare teams to ensure holistic care for patients.
  4. Utilize knowledge of social work principles and techniques to address a wide range of patient needs.
  5. Identify and connect patients and families with appropriate community resources and support services.
  6. Maintain accurate and thorough documentation of patient interactions, assessments, and interventions.
  7. Advocate for patients' rights and needs within the healthcare system.
  8. Participate in interdisciplinary team meetings to discuss patient care and make recommendations.
  9. Stay up-to-date on best practices and current research in the field of social work.
  10. Adhere to ethical and professional standards in all interactions and interventions.
  11. Provide crisis intervention and support for patients and families in difficult situations.
  12. Collaborate with insurance companies and other agencies to coordinate care and services for patients.
  13. Participate in ongoing training and professional development opportunities.
  14. Maintain confidentiality and adhere to HIPAA regulations in all aspects of the job.
  15. Serve as a resource and mentor to other social work staff members.

Job Qualifications
  • A Master's Degree In Social Work From An Accredited Institution.

  • A Current And Valid Clinical Social Worker Associate (Cswa) Or Licensed Clinical Social Worker (Lcsw) License In The State Of Oregon.

  • Minimum Of 2 Years Of Experience In A Clinical Setting, Preferably In A Healthcare Or Hospital Setting.

  • Strong Understanding Of Evidence-Based Practices And Knowledge Of Mental Health Diagnoses And Treatment.

  • Excellent Communication And Interpersonal Skills, With The Ability To Work Collaboratively With A Multidisciplinary Team And Provide Culturally Sensitive Care To Diverse Patient Populations.
