Job Description

  1. Develop and implement comprehensive wound care plans for patients, in collaboration with the interdisciplinary team, to ensure the highest quality of care is provided.
  2. Monitor and assess patient's wound healing progress, and make necessary adjustments to treatment plans based on clinical observations.
  3. Educate and train staff on wound care best practices, including prevention, assessment, and treatment of wounds.
  4. Collaborate with healthcare providers to ensure timely and appropriate interventions for wound care patients.
  5. Maintain accurate and complete records of wound care treatments and progress for each patient.
  6. Stay updated on current wound care research, treatments, and best practices to ensure the delivery of evidence-based care.
  7. Serve as a resource for patients and their families, providing education and support on wound care management and prevention.
  8. Work closely with suppliers and vendors to ensure the availability of necessary wound care supplies and equipment.
  9. Conduct regular audits and quality assurance checks to ensure compliance with wound care standards and protocols.
  10. Participate in interdisciplinary team meetings and provide updates on wound care patients' progress and treatment plans.

Job Qualifications
  • Bachelor's Degree In Nursing Or Related Field: A Wound Care Coordinator At Kindred Healthcare Must Possess A Bachelor's Degree In Nursing Or A Related Field. This Educational Background Provides The Necessary Knowledge And Skills In Wound Care Management And Treatment.

  • Wound Care Certification: Candidates For The Wound Care Coordinator Position At Kindred Healthcare Should Have A Certification In Wound Care, Such As The Certified Wound Care Nurse (Cwcn) Or The Wound, Ostomy, And Continence Nurse (Wocn) Certification. This Demonstrates A High Level Of Expertise In Wound Care Management.

  • Minimum Of 3 Years Of Experience In Wound Care: Kindred Healthcare Requires Applicants For The Wound Care Coordinator Role To Have At Least 3 Years Of Experience In Wound Care. This Ensures That The Candidate Has A Strong Foundation In Wound Care Assessment, Treatment, And Management.

  • Experience In Leadership Or Management: As A Wound Care Coordinator, The Individual Will Be Responsible For Overseeing A Team Of Wound Care Nurses And Collaborating With Other Healthcare Professionals. Therefore, Kindred Healthcare Looks For Candidates With Experience In Leadership Or Management Roles.

  • Strong Communication And Interpersonal Skills: The Wound Care Coordinator At Kindred Healthcare Will Be Working Closely With Patients, Families, And Healthcare Providers. As Such, Excellent Communication And Interpersonal Skills Are Essential For This Role. This Includes The Ability To Provide Education And Support To Patients And Their Families, As Well As Effectively Communicate With The Healthcare Team.
