
Wound Care Coordinator

- Develop and implement comprehensive wound care plans for patients, in collaboration with the interdisciplinary team, to ensure the highest quality of care is provided.
- Monitor and assess patient's wound healing progress, and make necessary adjustments to treatment plans based on clinical observations.
- Educate and train staff on wound care best practices, including prevention, assessment, and treatment of wounds.
- Collaborate with healthcare providers to ensure timely and appropriate interventions for wound care patients.
- Maintain accurate and complete records of wound care treatments and progress for each patient.
- Stay updated on current wound care research, treatments, and best practices to ensure the delivery of evidence-based care.
- Serve as a resource for patients and their families, providing education and support on wound care management and prevention.
- Work closely with suppliers and vendors to ensure the availability of necessary wound care supplies and equipment.
- Conduct regular audits and quality assurance checks to ensure compliance with wound care standards and protocols.
- Participate in interdisciplinary team meetings and provide updates on wound care patients' progress and treatment plans.
Bachelor's Degree In Nursing Or Related Field: A Wound Care Coordinator At Kindred Healthcare Must Possess A Bachelor's Degree In Nursing Or A Related Field. This Educational Background Provides The Necessary Knowledge And Skills In Wound Care Management And Treatment.
Wound Care Certification: Candidates For The Wound Care Coordinator Position At Kindred Healthcare Should Have A Certification In Wound Care, Such As The Certified Wound Care Nurse (Cwcn) Or The Wound, Ostomy, And Continence Nurse (Wocn) Certification. This Demonstrates A High Level Of Expertise In Wound Care Management.
Minimum Of 3 Years Of Experience In Wound Care: Kindred Healthcare Requires Applicants For The Wound Care Coordinator Role To Have At Least 3 Years Of Experience In Wound Care. This Ensures That The Candidate Has A Strong Foundation In Wound Care Assessment, Treatment, And Management.
