- Develop and implement personalized nutrition plans for patients based on their individual needs and medical conditions.
- Conduct assessments and evaluations of patients' nutritional status and make recommendations for dietary changes or supplements as needed.
- Collaborate with interdisciplinary healthcare team to ensure patients' nutritional needs are being met.
- Provide education and support to patients and their families on proper nutrition and its impact on overall health.
- Maintain accurate and up-to-date records of patients' nutrition plans and progress.
- Monitor patients' food intake and make adjustments to their nutrition plans as needed.
- Stay current with industry research and updates in nutrition and apply new knowledge to patient care.
- Advocate for patients' nutritional needs and work to improve their overall well-being.
- Communicate effectively with patients, families, and healthcare team members to ensure coordinated and comprehensive care.
- Adhere to all relevant laws, regulations, and ethical standards related to nutrition and patient care.
- Participate in continuing education and professional development opportunities to maintain and enhance skills and knowledge.
- Maintain a positive, compassionate, and empathetic attitude towards patients and their families.
- Continuously evaluate and improve nutrition services at Kindred Healthcare.
- Foster a safe and welcoming environment for patients and their families.
- Handle any patient concerns or complaints related to nutrition services in a timely and professional manner.
Bachelor's Or Master's Degree In Dietetics Or A Related Field From An Accredited University.
Current Registration As A Registered Dietitian With The Commission On Dietetic Registration.
Minimum Of 1-2 Years Of Experience In Clinical Nutrition Or Healthcare Setting.
Strong Knowledge Of Medical Nutrition Therapy, Enteral And Parenteral Nutrition, And Diet-Related Disease Management.
Excellent Communication And Interpersonal Skills, With The Ability To Educate And Counsel Patients And Collaborate With Interdisciplinary Healthcare Team.
