- Dispensing medications accurately and efficiently in accordance with state and federal regulations.
- Verifying prescription orders and patient information to ensure safe and effective use of medications.
- Collaborating with healthcare team members to develop and implement comprehensive medication management plans for patients.
- Providing drug information and education to patients, family members, and healthcare staff.
- Monitoring patient medication therapy for efficacy and safety, making recommendations for adjustments as needed.
- Maintaining accurate and organized records of medication dispensing and patient medication profiles.
- Adhering to all medication storage and handling procedures to ensure quality and safety standards are met.
- Participating in quality improvement initiatives to promote safe and effective medication use.
- Staying up-to-date on current pharmacy practices, trends, and developments in the field.
- Providing exceptional customer service to patients and their families, addressing any questions or concerns regarding their medications.
- Collaborating with insurance companies and other third-party payers to ensure timely and accurate billing.
- Maintaining a clean and organized work environment, following all infection control and safety protocols.
- Adhering to all company policies, procedures, and regulations.
- Participating in continuing education and training programs to maintain and enhance clinical skills.
- Representing Kindred Healthcare in a professional and positive manner at all times.
Bachelor's Degree In Pharmacy Or Doctor Of Pharmacy (Pharm.d.) From An Accredited Institution.
Active And Unrestricted Pharmacist License In The State Where The Job Is Located.
Minimum Of 1-2 Years Of Experience As A Licensed Pharmacist, Preferably In A Hospital Or Long-Term Care Setting.
Advanced Knowledge Of Pharmaceuticals, Drug Therapies, And Medication Management.
Strong Communication And Interpersonal Skills, With The Ability To Work Collaboratively With Other Healthcare Professionals And Patients.
