
Patient Assessment Coordinator

- Conduct initial patient assessments to determine their care needs and develop individualized care plans.
- Monitor patients' progress and update care plans as needed.
- Communicate effectively with healthcare professionals, patients, and their families to coordinate care and ensure all parties are informed and involved in the treatment process.
- Maintain accurate and up-to-date patient records and documents.
- Collaborate with interdisciplinary teams to provide comprehensive care and address any issues or concerns.
- Proactively identify potential problems or barriers to care and work to find solutions.
- Advocate for patients' needs and rights, ensuring that they receive the best possible care and treatment.
- Stay current with industry standards, regulations, and best practices related to patient care and assessment.
- Ensure compliance with all relevant laws, regulations, and policies.
- Maintain confidentiality of patient information and adhere to HIPAA guidelines.
- Provide emotional support and guidance to patients and their families during difficult times.
- Participate in quality improvement initiatives to enhance patient care.
- Train and educate staff on proper assessment techniques and protocols.
- Attend meetings and training sessions as required.
- Other duties as assigned by management.
Bachelor's Degree In Nursing Or A Related Healthcare Field.
Minimum Of 2 Years Experience In A Clinical Setting, Preferably In Patient Assessment Or Case Management.
Knowledge Of Medicare And Medicaid Regulations And Guidelines.
Strong Communication Skills, Both Written And Verbal.
Ability To Multitask And Work In A Fast-Paced Environment.
