Job Description

As the Director, you will play a vital role in overseeing and leading our Case Management department to ensure the highest quality of care for our patients.   1. Oversee and lead the Case Management department to ensure efficient and effective operations.
  2. Develop and implement strategies to improve the quality of care for patients.
  3. Set and monitor department goals and objectives in alignment with the organization's overall mission and vision.
  4. Hire, train, and mentor Case Management staff to ensure a high-performing team.
  5. Create and maintain department policies and procedures in accordance with regulatory standards.
  6. Foster a positive and collaborative work environment that promotes teamwork, open communication, and professional growth.
  7. Collaborate with other departments and healthcare professionals to coordinate and optimize patient care.
  8. Monitor and analyze department performance metrics to identify areas for improvement and implement necessary changes.
  9. Ensure compliance with all applicable laws, regulations, and accreditation standards.
  10. Serve as a liaison between Case Management and other departments, as well as with external stakeholders.
  11. Develop and manage department budget and resources effectively.
  12. Stay updated on industry developments and trends in Case Management to continuously improve practices.
  13. Represent the organization in a professional and positive manner at internal and external events.
  14. Handle any escalated issues or concerns related to Case Management operations.
  15. Promote a patient-centered approach to care and ensure the highest level of satisfaction for patients and their families.
  16. Act as a role model for ethical and compassionate behavior in all interactions with patients, families, and staff.

Job Qualifications
  • Bachelor's Or Master's Degree In Nursing, Healthcare Administration Or Related Field.

  • Minimum Of 3-5 Years Of Experience In Case Management, Preferably In A Healthcare Setting.

  • Knowledge Of Medicare, Medicaid, And Third-Party Payer Regulations And Guidelines.

  • Excellent Communication And Leadership Skills.

  • Certification In Case Management (Ccm) Preferred.
