Bachelor's Degree In Social Work, Nursing, Or A Related Field.
Minimum Of 2 Years Of Experience Working As A Case Manager In A Healthcare Setting.
Knowledge Of State And Federal Regulations And Guidelines Related To Case Management And Healthcare.
Excellent Communication And Interpersonal Skills To Effectively Collaborate With Patients, Families, And Interdisciplinary Teams.
Strong Problem-Solving And Critical Thinking Skills To Develop Effective Care Plans And Address Complex Patient Needs.
