  1. Coordinate and manage the care of patients to ensure they receive the best possible treatment.
  2. Develop and implement individualized care plans for patients based on their specific needs and medical history.
  3. Collaborate with healthcare professionals, including physicians, nurses, and therapists, to ensure cohesive and comprehensive care for each patient.
  4. Monitor patient progress and make necessary adjustments to their care plan as needed.
  5. Advocate for patients and their families to ensure their needs and concerns are addressed.
  6. Conduct regular assessments and evaluations to determine the effectiveness of treatment plans and make recommendations for adjustments.
  7. Communicate with insurance companies and other healthcare providers to obtain necessary authorizations and referrals.
  8. Maintain accurate and detailed records of patient care and progress.
  9. Provide emotional support and guidance to patients and their families during difficult times.
  10. Stay up-to-date with industry developments and best practices to continuously improve patient care.
  11. Participate in interdisciplinary team meetings to discuss patient progress and make recommendations for care.
  12. Collaborate with community resources to arrange for post-discharge care and support for patients.
  13. Adhere to all applicable laws, regulations, and ethical standards in the delivery of patient care.
  14. Uphold the values and mission of Kindred Healthcare through all interactions with patients, families, and colleagues.
  15. Attend trainings and professional development opportunities to enhance skills and knowledge in case management.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work, Nursing, Or A Related Field.

  • Minimum Of 2-3 Years Of Experience In Case Management, Preferably In A Healthcare Setting.

  • Strong Communication And Interpersonal Skills To Effectively Interact With Patients, Families, And Healthcare Professionals.

  • Knowledge And Understanding Of Healthcare Regulations, Insurance Plans, And Discharge Planning Processes.

  • Ability To Multitask And Prioritize Tasks Effectively, While Maintaining Attention To Detail And Accuracy.
