  1. Develop and implement individualized care plans for patients based on their specific needs and goals.

  3. Advocate for patient needs and ensure that their preferences and goals are considered in their care plans.
  4. Monitor and evaluate patient progress and make necessary adjustments to care plans as needed.

  9. Collaborate with insurance providers and community resources to coordinate services and support for patients.
  10. Participate in staff meetings, trainings, and other professional development activities to continuously improve skills and knowledge.
  11. Serve as a resource and mentor to other healthcare professionals, providing guidance and support as needed.
  12. Maintain confidentiality and adhere to ethical standards in all interactions with patients and their families.
  13. Act as a liaison between patients, families, and healthcare providers to promote effective communication and understanding.
  14. Continuously evaluate and improve processes and procedures to enhance the quality and efficiency of patient care.
  15. Demonstrate a strong commitment to making a positive impact in the lives of others and promoting a patient-centered approach to care.
