Job Description

If so, Kettering Health Network is seeking a dedicated and empathetic Social Worker to join our Care Management Acute team. In this role, you will have the opportunity to provide essential support and resources to patients and their families during their hospital stay. We are looking for a highly motivated individual with exceptional communication and problem-solving skills, as well as a deep understanding of social work principles and ethics.   1. Conduct psychosocial assessments to identify the needs and concerns of patients and their families.
  2. Collaborate with interdisciplinary team members to develop and implement individualized care plans.
  3. Provide counseling and support to patients and families facing emotional, social, and financial challenges.
  4. Connect patients with appropriate community resources and assistance programs.
  5. Advocate for patients' rights and ensure they are receiving appropriate care.
  6. Work with insurance companies and other agencies to assist with discharge planning.
  7. Maintain accurate and thorough documentation of all interactions and interventions.
  8. Participate in case conferences and other team meetings to discuss patient progress and treatment plans.
  9. Educate patients and families about available resources and support services.
  10. Stay up-to-date on social work principles, ethics, and best practices.
  11. Maintain confidentiality and adhere to all HIPAA regulations.
  12. Provide crisis intervention and support during emergencies.
  13. Develop and maintain positive relationships with patients, families, and healthcare team members.
  14. Collaborate with community organizations and agencies to develop and improve services for patients.
  15. Continuously evaluate and improve social work services to meet the changing needs of patients and families.
  16. Participate in training and professional development opportunities to enhance skills and knowledge.
  17. Comply with all hospital policies and procedures.
  18. Adhere to ethical standards and maintain professional boundaries.
  19. Proactively identify and address potential barriers to patient care and provide appropriate interventions.
  20. Serve as a positive representative of the hospital and its mission to provide high-quality, compassionate care to the community.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work From An Accredited Institution.

  • Valid And Current State Licensure As A Social Worker.

  • Minimum Of 2 Years Of Experience In A Healthcare Setting, Preferably In Acute Care Or Care Management.

  • Strong Knowledge And Understanding Of Healthcare Systems And Resources, Including Insurance Coverage And Government Programs.

  • Excellent Communication, Organizational, And Problem-Solving Skills, With The Ability To Work Collaboratively In A Multidisciplinary Team.
