
Registered Nurse Community Wellness Patient Educator

- Conduct community wellness programs and workshops to educate patients and their families on various health topics.
- Develop and implement educational materials and resources for patients, tailored to their specific needs and conditions.
- Collaborate with healthcare providers to identify community health needs and develop targeted education programs.
- Provide one-on-one patient education and counseling to promote healthy lifestyle choices and disease prevention.
- Serve as a liaison between the healthcare facility and the community, building relationships and partnerships to enhance patient education and outreach efforts.
- Utilize evidence-based practices and current healthcare guidelines to deliver accurate and up-to-date information.
- Evaluate the effectiveness of community wellness programs and make necessary adjustments to improve outcomes.
- Maintain accurate records and documentation of patient education activities.
- Stay informed on new developments and advancements in healthcare to ensure the most relevant and accurate information is provided to patients.
- Serve as a role model for promoting healthy behaviors and self-care practices.
- Collaborate with other healthcare professionals to develop and implement comprehensive patient education plans.
- Maintain a professional and compassionate demeanor, actively listening to patient concerns and addressing them in a timely and effective manner.

Valid Rn License: A Qualified Candidate For This Position Must Possess A Valid Registered Nurse License In The State Of Ohio. This Ensures That They Have Met The Necessary Educational And Training Requirements To Provide Patient Education And Wellness Services.
Bachelor's Degree In Nursing: Kettering Health Network Requires All Registered Nurse Community Wellness Patient Educators To Have A Minimum Of A Bachelor's Degree In Nursing. This Degree Provides A Strong Foundation In Nursing Theory, Research, And Practice, Which Is Essential For This Role.
Experience In Community Health: Candidates Must Have Prior Experience In Community Health, Such As Working In A Community Clinic Or Providing Health Education Programs. This Ensures That They Are Familiar With The Unique Needs And Challenges Of Community-Based Healthcare.
Strong Communication And Teaching Skills: As A Patient Educator, The Candidate Must Have Excellent Communication And Teaching Skills To Effectively Convey Health Information To Patients And Community Members. This Includes The Ability To Explain Complex Medical Concepts In An Easy-To-Understand Manner.
Knowledge Of Health Promotion And Disease Prevention: The Ideal Candidate Should Have A Strong Understanding Of Health Promotion And Disease Prevention Strategies.
