  1. Provide compassionate and dedicated care to patients and their families throughout their healthcare journey at Johns Hopkins Medicine.
  2. Guide and advocate for patients by serving as the main point of contact and resource for navigating the healthcare system.
  3. Utilize strong clinical knowledge and expertise to assess, plan, implement, and evaluate patient care.
  4. Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients.
  5. Monitor and track patient progress, identifying any potential barriers to care and addressing them proactively.
  6. Educate patients and their families on treatment options, procedures, and medications, ensuring they have a thorough understanding of their healthcare plan.
  7. Maintain accurate and detailed patient records, documenting all interactions and interventions.
  8. Communicate effectively and empathetically with patients, families, and healthcare providers, ensuring clear understanding and coordination of care.
  9. Stay up-to-date on current healthcare trends, policies, and procedures to provide the best possible care for patients.
  10. Demonstrate exceptional interpersonal skills, building rapport and trust with patients and families to provide a positive healthcare experience.
  11. Participate in quality improvement initiatives to enhance patient care and outcomes.
  12. Adhere to all ethical, legal, and regulatory standards in the delivery of patient care.
  13. Continuously evaluate and improve processes and procedures to enhance the patient experience and streamline healthcare delivery.
  14. Collaborate with community resources and organizations to provide comprehensive care for patients.
  15. Maintain a positive and professional attitude, representing Johns Hopkins Medicine with integrity and commitment to excellence.

Job Qualifications
  • Valid Rn License: A Registered Nurse - Nurse Navigator At Johns Hopkins Medicine Must Possess A Valid Nursing License From The State Of Maryland.

  • Bachelor's Degree In Nursing: Candidates Should Have A Bachelor's Degree In Nursing (Bsn) From An Accredited Institution. A Higher Degree Such As A Master's In Nursing (Msn) May Also Be Preferred.

  • Clinical Experience: A Minimum Of 2-3 Years Of Clinical Experience As A Registered Nurse Is Required For This Position. Experience In A Specialty Area Such As Oncology, Cardiology, Or Pediatrics May Be Preferred.

  • Certification: Candidates Should Hold A Certification In Their Area Of Specialty, Such As Oncology Certified Nurse (Ocn) Or Certified Pediatric Nurse (Cpn). This Demonstrates A High Level Of Knowledge And Expertise In Their Field.

  • Communication And Interpersonal Skills: A Nurse Navigator Is Responsible For Coordinating Care And Communicating With Patients, Healthcare Providers, And Other Team Members. Excellent Communication And Interpersonal Skills Are Essential For This Role.
