- Develop and implement personalized treatment plans in collaboration with physicians, nurses, and other healthcare providers.
- Monitor medication therapy for effectiveness, safety, and appropriateness.
- Provide medication education to patients and their families, including instructions on proper use, potential side effects, and drug interactions.
- Conduct medication reconciliation to ensure accuracy and continuity of care.
- Participate in multidisciplinary rounds and provide clinical recommendations.
- Serve as a drug information resource for healthcare providers.
- Conduct medication therapy management and make recommendations for optimization.
- Identify and resolve medication-related problems.
- Collaborate with the pharmacy team to ensure timely and accurate medication dispensing.
- Participate in quality improvement initiatives related to medication use.

- Maintain accurate and complete patient records.
- Adhere to all state and federal laws and regulations related to pharmacy practice.
- Act as a preceptor for pharmacy residents and students.

Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution
Completion Of A Post-Graduate Pharmacy Residency Program In Clinical Pharmacy Or Related Field
Board Certification In A Relevant Area Of Practice (E.g. Pharmacotherapy, Ambulatory Care, Critical Care)
Active Pharmacist License In The State Of Maryland (Or Eligibility For Licensure)
