- Develop and implement medication therapy plans for surgical patients in collaboration with the healthcare team.
- Conduct medication reconciliation and review for surgical patients to ensure accurate and safe medication use.
- Provide drug information and education to healthcare team members, patients, and their families regarding medication therapy and potential drug interactions.
- Monitor patient response to medication therapy and adjust treatment plans as needed.
- Evaluate medication orders for appropriateness and accuracy, including dose, route, and frequency.
- Collaborate with surgeons and other healthcare providers to develop protocols and guidelines for medication use in surgical patients.
- Keep abreast of current developments and advances in the field of surgery and pharmacy, and incorporate new information into practice.
- Ensure compliance with all state and federal regulations related to medication use and storage.
- Participate in quality improvement initiatives and medication safety programs.
- Maintain accurate and complete patient records and documentation related to medication therapy.
- Serve as a resource for other healthcare team members regarding medication therapy and drug-related issues.
- Train and supervise pharmacy students and residents in the area of surgical pharmacy.
- Participate in multidisciplinary rounds and meetings to provide input on medication-related issues.
- Collaborate with hospital staff to develop and implement medication management protocols and procedures.
- Maintain a high level of professionalism and adhere to ethical standards in all interactions with patients, colleagues, and healthcare organizations.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution: A Clinical Pharmacist At Johns Hopkins Medicine Must Have Completed A Rigorous Pharmd Program From An Accredited University In Order To Be Considered For This Position.
Residency Training In A Surgical Or Critical Care Setting: A Specialized Residency Program In Surgery Or Critical Care Is Highly Desirable For A Clinical Pharmacist At Johns Hopkins Medicine To Gain The Necessary Experience And Expertise In This Field.
Board Certification In Pharmacotherapy Or Critical Care: Candidates With Board Certification In Pharmacotherapy Or Critical Care Demonstrate A High Level Of Knowledge And Expertise In Medication Management, Making Them Ideal Candidates For A Clinical Pharmacist Position In A Surgical Setting.
Experience In A Surgical Or Critical Care Setting: Prior Experience Working As A Pharmacist In A Surgical Or Critical Care Setting Is Essential For This Position At Johns Hopkins Medicine.
