  4. Monitor patient responses to medications and adjust dosages as needed.
  5. Educate patients and their families about their medications, including proper administration and potential side effects.
  6. Stay up-to-date on the latest developments in medicine and hepatology to ensure the best possible care for patients.
  7. Participate in multidisciplinary team meetings and provide expertise on medication-related issues.
  8. Maintain accurate and complete patient records, including medication histories and treatment plans.
  9. Adhere to all hospital policies and procedures related to medication management and patient care.
  10. Collaborate with pharmacy technicians and other staff members to ensure efficient and accurate medication dispensing.
  11. Use technology and software systems to manage patient medication orders and records.
  12. Comply with all regulatory requirements and maintain a safe and clean work environment.
  13. Continuously evaluate and improve the medication management process to enhance patient outcomes.
  14. Serve as a resource for other healthcare professionals and provide drug information when needed.
  15. Attend relevant seminars, conferences, and training sessions to stay current in the field of medicine and hepatology.

Job Qualifications
  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited College Of Pharmacy.

  • Completion Of A Post-Graduate Residency Or Fellowship In Clinical Pharmacy, With A Focus On Medicine And Hepatology.

  • Active Licensure As A Pharmacist In The State Of Maryland, Or Eligibility For Licensure.

  • Minimum Of 3-5 Years Of Experience As A Clinical Pharmacist In A Hospital Or Academic Medical Center Setting, With A Demonstrated Expertise In Medicine And Hepatology.

  • Strong Communication And Collaboration Skills, Ability To Work Effectively Within A Multidisciplinary Team, And A Commitment To Evidence-Based Practice And Continuous Learning.
