
Case Manager - Transplant/Renal

- Coordinate and manage the care of patients undergoing transplant procedures or managing renal conditions.
- Work closely with a multidisciplinary team to support and advocate for patients and their families.
- Develop individualized care plans for patients and regularly review and update them.
- Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure the best possible outcomes for patients.
- Conduct comprehensive assessments of patients' physical, emotional, and psychosocial needs.
- Educate patients and their families on treatment options and post-treatment care.
- Monitor patients' progress and provide ongoing support and guidance.
- Identify and address any barriers to care and develop solutions to overcome them.
- Ensure timely and appropriate communication with patients, families, and healthcare team members.
- Document all patient interactions and interventions accurately and thoroughly.
- Stay up-to-date with current transplant and renal care practices and guidelines.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Maintain confidentiality and adhere to all HIPAA regulations.
- Act as a resource and advocate for patients and their families.
- Attend meetings and trainings as required by the department and organization.
- Collaborate with external organizations and resources to provide comprehensive care for patients.
- Adhere to all hospital policies and procedures.
- Maintain a positive and professional attitude at all times.
- Serve as a role model for ethical and compassionate care.
- Perform other duties as assigned by the supervisor.
Bachelor's Or Master's Degree In Social Work, Nursing, Or Related Field.
Minimum Of 2-3 Years Of Experience In Case Management, Preferably In A Transplant Or Renal Setting.
Knowledge And Understanding Of Transplant And Renal Care Processes And Protocols.
Strong Communication And Interpersonal Skills To Effectively Work With Patients, Families, And Interdisciplinary Teams.
Certification In Case Management (Ccm) Or Transplant Coordination (Cctc) Preferred.
