
Adult Home Health Physical Therapist
At Johns Hopkins Medicine, we are seeking an Adult Home Health Physical Therapist to join our team and help our patients reach their highest physical potential. The ideal candidate for this position will be an experienced physical therapist with a passion for helping patients with adult home health needs. The successful candidate must have a strong commitment to providing quality care and have the ability to work independently and collaboratively with other clinical staff. As part of our team, the Adult Home Health Physical Therapist will be responsible for providing comprehensive physical therapy services in the home care setting.To be considered for this role, the successful candidate must possess a Bachelor’s Degree in Physical Therapy from an accredited college or university and have current licensure as a Physical Therapist in the state of Maryland. In addition, one year of home health physical therapy experience is preferred, but not required. The right candidate must also have excellent communication skills and the ability to build strong relationships with patients, families, and other members of the healthcare team. Finally, the successful candidate will need to demonstrate a commitment to providing quality care and strive for the highest standard of excellence.
Responsibilities of the Adult Home Health Physical Therapist:
- Provide comprehensive physical therapy services to home care patients.
- Develop individualized treatment plans for patients based on their individual needs.
- Monitor progress and modify treatment plans as needed.
- Communicate effectively with patients, families, and other members of the healthcare team.
- Maintain current licensure in the state of Maryland.
- Ensure the highest standard of excellence in patient care.
- Adhere to all applicable laws and regulations.
- Maintain patient confidentiality at all times.
- Attend continuing education classes to stay up to date on best practices.
- Document all treatments accurately and efficiently.
