Adult Home Care RN

Johns Hopkins Medicine is seeking an experienced Adult Home Care RN to provide outstanding care and support to our home care patients. The successful candidate will be a highly motivated, compassionate individual who is committed to providing quality care and ensuring the best outcomes for our patients.The Adult Home Care RN at Johns Hopkins Medicine will be responsible for providing nursing care to patients in their homes, including comprehensive assessments, care planning, and patient/family education. In this role, the RN will collaborate with other disciplines and departments to provide the highest quality of care for our patients.The ideal candidate for this position will have a degree in nursing from an accredited program, current RN licensure in the state of Maryland, and at least three years of nursing experience, preferably in a home care setting. The successful candidate will also have strong written and verbal communication skills, the ability to interact with patients of all ages, and a commitment to providing excellent patient care.

Responsibilities:

1. Conduct comprehensive assessments and create care plans for home care patients.
2. Educate and support patients and their families throughout the home care process.
3. Collaborate with other healthcare professionals to ensure the highest quality of care for patients.
4. Administer medications and treatments as prescribed by physicians.
5. Monitor patient health and progress, and maintain appropriate documentation.
6. Respond to patient inquiries and concerns in a timely manner.
7. Maintain current RN licensure in the state of Maryland.
8. Remain up-to-date with best practices and developments in home care nursing.
9. Ensure compliance with all regulatory and legal standards.
