- Provide exceptional and compassionate care to patients during their transition from acute care to home or other healthcare settings.
- Collaborate with healthcare team members to develop and implement individualized care plans for patients.
- Conduct thorough assessments of patients' physical and emotional needs.
- Monitor patients' progress and make necessary adjustments to their care plans.
- Educate patients and their families on post-discharge care instructions and resources.
- Communicate effectively with patients, families, and healthcare team members to ensure a smooth transition process.
- Ensure the accuracy and completeness of patients' medical records.
- Administer medications and treatments as prescribed by physicians.
- Identify and report any changes in patients' conditions to the appropriate healthcare team members.
- Maintain a safe and clean environment for patients and staff.

Current Rn License: The Rn Transition In Practice Role At Huntington Hospital Requires A Valid And Active Rn License In The State Of California. This Ensures That The Candidate Has Met All The Necessary Educational And Training Requirements To Practice As A Registered Nurse.
Bachelor's Degree In Nursing: Candidates For This Role Should Possess A Bachelor's Degree In Nursing From An Accredited Institution. This Degree Provides A Strong Foundation In Nursing Theory And Practice, And Is Increasingly Becoming The Preferred Qualification For Registered Nurses.
Minimum Of 2 Years Of Clinical Experience: The Rn Transition In Practice Role At Huntington Hospital Is A Specialized Position That Requires A Minimum Of 2 Years Of Experience As An Rn In A Clinical Setting.
