- Maintain confidentiality and adhere to ethical standards in all aspects of the job.
- Attend and participate in team meetings, trainings, and continuing education opportunities.
- Collaborate with community resources and agencies to provide additional support for patients and families.
- Adhere to hospital policies and procedures, including quality and safety standards.
- Use effective time management skills to handle a fast-paced workload and prioritize tasks.

Bachelor's Degree In Social Work, Nursing, Or A Related Field.
Minimum Of 2 Years Of Experience In Case Management Or Healthcare Coordination.
Knowledge Of Healthcare Systems, Insurance Plans, And Discharge Planning Processes.
Strong Communication And Interpersonal Skills To Work Effectively With Patients, Families, And Healthcare Professionals.
