  1. Coordinate and oversee the care of patients, ensuring their needs are met and their treatment plans are effectively managed.
  2. Serve as an advocate for patients, working closely with healthcare providers and other stakeholders to ensure the best possible outcomes.
  3. Utilize strong background in case management to assess and evaluate patient needs and develop individualized care plans.
  4. Collaborate with interdisciplinary healthcare team members to coordinate and facilitate patient care.
  5. Communicate effectively with patients, families, and caregivers to educate them on treatment plans and provide support.
  6. Utilize problem-solving skills to identify and address barriers to patient care and develop solutions.
  7. Conduct regular assessments of patient progress and make necessary adjustments to care plans.
  8. Maintain accurate and thorough documentation of patient care and progress.
  9. Stay up-to-date on relevant healthcare policies and regulations to ensure compliance.
  10. Participate in discharge planning and provide resources for patients and families to ensure a smooth transition to post-hospital care.
  11. Provide emotional support and resources for patients and families during difficult times.
  12. Serve as a liaison between patients, healthcare providers, and insurance companies to ensure appropriate coverage and reimbursement.
  13. Maintain confidentiality and adhere to ethical standards in all aspects of the job.
  14. Attend and participate in team meetings, trainings, and continuing education opportunities.
  15. Collaborate with community resources and agencies to provide additional support for patients and families.
  16. Adhere to hospital policies and procedures, including quality and safety standards.
  17. Use effective time management skills to handle a fast-paced workload and prioritize tasks.
  18. Maintain a positive and professional attitude while working with patients, families, and colleagues.
  19. Continuously evaluate and improve processes to optimize patient care and outcomes.
  20. Represent the organization in a positive manner and promote its mission and values.

Job Qualifications
