Job Description

At HOAG, we are looking for a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our home health team. Our ideal candidate will bring experience, enthusiasm, and a commitment to providing the highest quality of care to our patients. This is a great opportunity for an LVN who is eager to make a difference in the lives of our patients and their families.To be considered for this position, you will need to have a valid LVN license in the state of California, the ability to work independently, and at least one year of experience in home health or hospice care. Responsibilities:

  1. Provide compassionate, empathetic, and quality care for home health patients and their families.
  2. Administer medications, treatments, and therapies as prescribed by physician.
  3. Monitor patients’ vital signs and health condition.
  4. Develop individualized care plans for each patient.
  5. Educate patients and family members on treatment plans and lifestyle changes.
  6. Maintain accurate documentation and records of patients’ care.
  7. Coordinate with other healthcare professionals to provide integrated care.
  8. Follow safety regulations and standards of practice.
  9. Maintain valid LVN license in the state of California.
  10. Possess valid driver’s license and reliable transportation.
