  1. Conduct assessments and develop treatment plans for patients experiencing trauma in an inpatient setting.
  2. Provide emotional support and counseling to patients and their families to help them cope with the effects of trauma.
  3. Collaborate with interdisciplinary teams to develop and implement comprehensive care plans that address the individual needs of each patient.
  4. Coordinate and facilitate communication between patients, families, and healthcare providers to ensure continuity of care.
  5. Advocate for patients' rights and needs, and ensure that their physical, emotional, and social needs are being met.
  6. Utilize evidence-based interventions and techniques to effectively address the impact of trauma on patients' lives.
  7. Maintain accurate and timely documentation of patients' progress and treatment plans.
  8. Provide crisis intervention and support to patients experiencing acute distress or mental health crises.
  9. Keep up-to-date with current research, best practices, and policies related to trauma-informed care.
  10. Participate in regular team meetings and case conferences to discuss patients' progress and collaborate on treatment plans.
  11. Provide education and resources to patients and their families to promote self-care and coping strategies.
  12. Adhere to ethical and professional standards of practice and maintain confidentiality in all aspects of the job.
  13. Engage in ongoing professional development and training to enhance skills and knowledge in the field of social work and trauma-informed care.
  14. Demonstrate a strong commitment to the values and mission of the organization, and actively contribute to creating a positive and supportive work environment.
  15. Serve as a liaison between the organization and community resources, and establish partnerships to enhance the care and support provided to patients.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work Or Related Field.

  • Minimum Of 2-3 Years Experience In Case Management, Preferably In An Inpatient Trauma Setting.

  • Knowledge Of Trauma-Informed Care And Ability To Assess And Address The Unique Needs Of Trauma Survivors.

  • Strong Communication And Interpersonal Skills, With The Ability To Work Collaboratively With Interdisciplinary Teams And Communicate Effectively With Clients And Their Families.

  • Understanding Of Insurance And Healthcare Systems, Including Knowledge Of Eligibility And Authorization Processes, To Ensure Appropriate Utilization Of Resources For Clients.
