Current Rn License: The Candidate Must Hold A Valid And Active Registered Nurse License In The State Where They Will Be Practicing.
Bachelor's Degree In Nursing: A Minimum Of A Bachelor's Degree In Nursing Is Required For This Role. A Higher Level Of Education, Such As A Master's Degree, May Also Be Preferred.
Case Management Certification: The Candidate Should Have A Certification In Case Management, Such As The Certified Case Manager (Ccm) Credential, To Demonstrate Their Knowledge And Skills In This Area.
Clinical Experience: Previous Experience As A Registered Nurse, Preferably In A Case Management Or Care Coordination Role, Is Essential. The Candidate Should Have A Strong Understanding Of Healthcare Systems And Clinical Processes.
Communication And Interpersonal Skills: As A Case Manager, The Candidate Will Be Working Closely With Patients, Families, And Other Healthcare Professionals. Strong Communication And Interpersonal Skills Are Essential To Effectively Coordinate Care And Build Relationships With All Stakeholders.
