  1. Develop and implement provider transformation strategies to improve healthcare outcomes and performance.
  2. Conduct analysis of provider data to identify areas for improvement and develop action plans.
  3. Collaborate with internal and external stakeholders to identify and address barriers to provider performance.
  4. Monitor and track provider performance metrics, providing regular updates and recommendations for improvement.
  5. Train and educate providers on best practices and processes to improve their performance.
  6. Build and maintain positive relationships with providers, serving as a primary point of contact for any performance-related concerns.
  7. Conduct site visits and audits to evaluate provider performance and identify opportunities for improvement.
  8. Utilize data analytics to identify trends and patterns in provider performance and make data-driven recommendations.
  9. Develop and deliver presentations and reports to senior leadership on provider performance and transformation initiatives.
  10. Stay up-to-date on industry trends and best practices in healthcare performance improvement.
  11. Act as a change agent, promoting a culture of continuous improvement and transformation within the provider network.
  12. Collaborate with cross-functional teams to ensure alignment and integration of provider transformation efforts with overall company goals and strategies.

Job Qualifications
  • Bachelor's Degree In Nursing Or Related Field

  • Valid Registered Nurse (Rn) License In The State Of Pennsylvania Or Delaware

  • Minimum Of 3-5 Years Of Experience In Healthcare Industry, With Specific Experience In Provider Transformation And Healthcare Delivery Systems

  • Strong Understanding Of Healthcare Laws, Regulations, And Industry Trends, Particularly In The Eastern Pa And Delaware Regions

  • Excellent Communication And Interpersonal Skills, With The Ability To Build Relationships And Collaborate With Diverse Stakeholders, Including Providers, Executives, And Community Organizations.
