Job Description

In this role, you will serve as a liaison between patients, their families, and healthcare providers, guiding them through the complex healthcare system and helping them navigate their care journey.   1. Serve as a primary point of contact for patients, their families, and healthcare providers, establishing strong relationships and providing compassionate support.
  2. Guide patients through the complex healthcare system, including scheduling appointments, coordinating care with multiple providers, and navigating insurance and financial processes.
  3. Advocate for patients' needs and preferences, ensuring they receive the best possible care and treatment.
  4. Communicate effectively and regularly with patients, families, and healthcare providers to provide updates on patients' care plans and address any concerns or issues.
  5. Utilize strong organizational skills to maintain accurate and detailed records of patients' care journeys, including appointments, treatments, and outcomes.
  6. Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients, incorporating their preferences and goals.
  7. Stay up-to-date on healthcare policies, procedures, and regulations to ensure compliance and provide accurate information to patients and families.
  8. Demonstrate empathy and understanding in all interactions with patients and families, recognizing and addressing their emotional and psychological needs.
  9. Attend ongoing training and development opportunities to enhance knowledge and skills related to patient care and navigation.
  10. Continuously evaluate and improve processes to enhance the patient experience and outcomes.

Job Qualifications
  • Bachelor's Degree In A Healthcare-Related Field, Such As Nursing, Social Work, Or Public Health.

  • Minimum Of 2 Years Of Experience In A Healthcare Setting, Preferably In Patient Navigation, Care Coordination, Or Case Management.

  • Strong Communication And Interpersonal Skills, With The Ability To Effectively Advocate For Patients And Collaborate With A Multidisciplinary Team.

  • Knowledge Of Healthcare Systems, Insurance Coverage, And Resources Available To Patients, As Well As Understanding Of Healthcare Disparities And Cultural Competence.

  • Proficiency In Electronic Medical Records And Other Computer Systems, As Well As Data Analysis And Reporting Skills.
