Job Qualifications
  • Bachelor's Degree In Nursing, Social Work, Or A Related Field.

  • Minimum Of 2 Years Of Experience In Medical Case Management, Preferably In A Healthcare Setting.

  • Knowledge Of Delaware's Healthcare System And Resources, Particularly In New Castle County.

  • Strong Communication And Interpersonal Skills To Collaborate With Healthcare Providers, Patients, And Their Families.

  • Ability To Navigate And Document In Electronic Medical Records And Other Healthcare Databases.
