  1. Coordinate and oversee the implementation of health management programs.
  2. Provide support to program participants, including scheduling appointments, answering questions, and addressing concerns.
  3. Collaborate with healthcare professionals, including physicians, nurses, and social workers, to ensure effective and efficient delivery of services.
  4. Monitor and track program outcomes and make recommendations for improvement.
  5. Educate and inform program participants about available resources and services to help them achieve their health goals.
  6. Maintain accurate and up-to-date records of program participants and their progress.
  7. Develop and maintain relationships with community organizations and resources to enhance program offerings.
  8. Stay up-to-date on industry trends and best practices in health management and make recommendations for program enhancements.
  9. Conduct outreach and engage with potential program participants to promote the benefits of health management programs.
  10. Collaborate with team members to develop and implement marketing strategies to increase program participation.
  11. Maintain confidentiality and adhere to all HIPAA regulations in handling sensitive member information.
  12. Participate in training and professional development opportunities to enhance skills and knowledge.
  13. Follow company policies and procedures to ensure compliance with regulations.
  14. Provide exceptional customer service to program participants and promote a positive image of the company.
  15. Support company values and mission to improve the health and well-being of the community.

Job Qualifications
  • Bachelor's Degree In Healthcare Administration, Business Management, Or A Related Field.

  • Minimum Of 3 Years Experience In Healthcare Management Or Related Field.

  • Strong Understanding Of Healthcare Regulations And Compliance.

  • Excellent Organizational And Project Management Skills.

  • Ability To Effectively Communicate With Diverse Stakeholders Including Healthcare Providers, Insurance Companies, And Government Agencies.
