  1. Coordinate and implement various health management initiatives to promote the overall health and well-being of members.
  2. Develop and maintain relationships with healthcare providers and community organizations to enhance the effectiveness of health management programs.
  3. Monitor and track the progress of members enrolled in health management programs and provide support and guidance as needed.
  4. Collaborate with interdisciplinary teams to develop individualized care plans for members with complex health conditions.
  5. Conduct health assessments and screenings to identify potential health risks and develop appropriate interventions.
  6. Educate members on healthy lifestyle choices and disease prevention strategies.
  7. Utilize data and analytics to evaluate the effectiveness of health management programs and make recommendations for improvement.
  8. Serve as a resource and advocate for members, providing guidance and assistance in navigating the healthcare system.
  9. Stay current on industry trends and research related to health management and implement best practices.
  10. Maintain accurate and confidential records of member interactions and program outcomes.
  11. Collaborate with other departments within the organization to ensure a seamless and coordinated approach to member care.
  12. Act as a liaison between members and their healthcare providers, facilitating communication and promoting continuity of care.
  13. Participate in quality improvement initiatives to enhance the overall quality and effectiveness of health management programs.
  14. Adhere to all ethical and legal standards in the delivery of health management services.
  15. Maintain a positive and professional attitude while representing Highmark Health and its mission to improve the health and well-being of members and communities.

Job Qualifications
  • Bachelor's Degree In Healthcare Administration, Business Management, Or A Related Field.

  • Minimum Of 3-5 Years Of Experience In Healthcare Management Or Administration.

  • Knowledge Of Healthcare Laws And Regulations, Particularly In Regards To Insurance And Reimbursement.

  • Strong Communication And Interpersonal Skills, With The Ability To Collaborate And Build Relationships With Various Stakeholders.

  • Excellent Organizational And Project Management Skills, With The Ability To Effectively Prioritize Tasks And Manage Multiple Projects Simultaneously.
